Pharmacovigilance

Alpen Pharma priority is to take care of the health of patients. Therefore, gathering of information related to the efficacy and safety of medicinal products distributed by the Company is one of the core parts of our work. For this purpose, we’ve created a system of pharmacovigilance in the Polish Representative Office of Alpen Pharma.

In case if any suspected side effects after usage of medications for which Alpen Pharma is MAH Representative in Poland, please inform us about it by any of listed below ways:

  • By post: fill in the questionnaire available on this page (PDF file). You should print the questionnaire available on this page (PDF file), fill it in and send to us by post adressed on: Alpen Pharma Polsca Sp. z. o. o., Al. Jerozolimskie 151 lok. 1, 03-326 Warszawa.
  • By fax: fill in the questionnaire available on this page (PDF file). You should print the questionnaire available on this page (PDF file), fill it in and send to us by fax using the number +48 22 822 22 16.  
  • By phone: contact us by phone using the number +48 22 822 22 14.

If you observe any bothering symptoms after usage of medications you should contact your doctor immediately.  

We inform you, that all personal data collected due to receiving of your  questionnaire will be processed by Alpen Pharma Polsca Sp. z. o. o., z siedzibą w Warszawie przy Al. Jerozolimskich 151 lok. 1, under the rule of law Personal Data Protection  Act  of 29 Aug 1997 r. (Law Gazette from 2002, No 101, heading 926, as amended) only for the reasons of fulfilling the obligation of medications safety monitoring. Each person has right to access its own personal data content and right to correct them.   Acknowledgement of personal data  is obligatory for persons obliged to report side effects and is a result of law regulations – Pharmaceutical Law, Act of  6 Sept 2001 r. (Law Gazette from 2008, No 45, heading 271 as amended) and from other law regulations. In case of any other persons - acknowledgement of personal data  is voluntary, but necessary for the acceptance of  questionnaire (side effects report).
